student-athletes! We hope that you are
all having a safe and healthy summer.
On this page, you will find the information that is required by the Athletic Training Staff. Please note that the paperwork has changed from previous years.
Paperwork can be located through student-athlete banner accounts.
Direction to Banner:
Go to my.bryant.edu.
Enter username (Bryant email) and password.
Click on Banner icon.
Click on Student Services and Financial Aid.
Click on Student-Athlete Forms.
WHAT YOU NEED
Returning Student-Athletes: Physical Exam Form, copy of insurance card (front and back), Banner paperwork, ADD/ADHD documentation (if applicable).
Freshman/Transfers: Physical Exam Form, copy of insurance card (front and back), sickle cell screen test results, proof of immunizations, Banner paperwork, ADD/ADHD documentation (if applicable). Orthopedic evaluations are required and must be performed at Bryant ($35 fee). This is not optional and cannot be completed prior to arrival on campus.
Student-Athletes may purchase Bryant University health insurance if student does not have adequate insurance. For more information, click here. All student-athletes MUST either waive or enroll in this option in banner without exception. (Option provided in banner paperwork under "Statement of Insurance".)
